Introduction

Liposuction has become one of the most commonly performed cosmetic surgery procedures all over the world. Evolution of liposuction has seen refinements in technique and improvement of patient safety related standards of care. Liposuction relies on an awareness of 3D anatomy of fat layers. Subcutaneous fat is divided into superficial, intermediate and deep.The majority of liposuction should be performed on the middle or deeper adipose layer. In fact, it is the preservation of the superficial adipose layer that is crucial in preventing skin irregularities including dimpling, waviness, hyperpigmentation, burns, and contour irregularities.

A variety of techniques have been described:
  1. Suction-assisted liposuction: It removes fat cells other tissues through mechanical avulsion
  2. Ultrasound-assisted liposuction: It is performed in 3 stages :tumescent anaesthesia  infiltration , UAL treatment to emulsify fat from deep to superficial layers and aspiration through suction cannula.It is of advantage in patients with saggy skin, fibrous areas and secondary surgeries.
  3. Power-assisted liposuction : It uses the reciprocating motion of the tip of the cannula to facilitate fat removal. It is advantageous in large volume liposuction as it reduces operator fatigue drastically.
  4. Laser assisted liposuction: It uses laser energy to liquefy the fat which is then aspirated via suction cannulas. The liquefying process is a bit more time consuming than other methods. 

Grading

 
  • Type 1: Localized lipodystrophy
    Younger patients with good skin tone and minimal skin irregularities.

Type 2: Generalized lipodystrophy
Slightly diminished skin tone with some skin irregularities and circumferential lipodystrophy throughout their trunk and extremities.

Type 3: Skin redundancy and lipodystrophy
Patients displaying significant skin redundancy that would be more amenable to excisional surgical techniques to improve shape and contour. Liposuction is used as an adjunct for optimal result

Consultation

You are required to visit the clinic for a face to face consultation with Dr Ashwani Kumar. You can discuss about various issues or doubts about the surgery. The body areas you want to be treated will be examined and a plan of action, number of sittings required will be discussed with you.

You will be required to undergo a few blood tests and imaging investigations to assess your fitness.

Surgery

You are called fasting on the day of surgery. The procedure is done under general anaesthesia. The duration of surgery depends on areas being treated. You will be kept under observation for 6 hours after surgery and then you will be discharged wearing a compression garment.

Sutures will be removed
